                                     Waiver of Liability form for Land-O-Fun

In Consideration of participation in any event, activity, and/or class at Land-O-Fun in Brooklyn NY. the undersigned, on his or her behalf and on the behalf of the participant(s) identified below, acknowledges, appreciates and agrees to the following conditions:

I represent that I am the parent or legal guardian of the participant(s) named below, or I have obtained permission from the parent/legal guardian of the participant(s) named below to execute this agreement on their behalf.

As an attendee of any party, activity or other event at Land-O-Fun, I acknowledge that there are dangers and risks associated with the activities and agree to assume all risks of personal injury, including the potential of paralysis and death. In case of injury, death, disability or loss/damage of personal property, Land-o-Fun, its owners, members, officers and/or employees will not be held responsible.

I, for myself and the participants named below, agree to follow the safety instructions, both verbal and posted, provided and acknowledge that failure to do so may result in removal from Land-O-Fun. If I am aware of any hazards, whether by other members or equipment at the facility, I will alert an employee of Land-O-Fun immediately.

I, for myself and the participants named below, and our respective heirs, assigns, personal representatives and next of kin, hereby hold harmless Land-O-Fun, its owners, members, officers, employees and sponsoring agencies from all liability for any such personal injury, disability, death or loss of damage to person or property to the fullest extent of the law.

SOCKS ARE REQUIRED FOR ALL PARTICIPANTS
